Opportunity in St. Louis, MO We are looking for a full-time General
Practice Physician to deliver comprehensive healthcare services to
patients in skilled nursing facilities as part of a
multidisciplinary post-acute care team in St. Louis, Missouri. This
role includes conducting patient evaluations, formulating
personalized care plans, coordinating treatment, and collaborating
with other healthcare professionals to enhance patient outcomes.
The position provides competitive pay, opportunities for

and empathetic physician to join our post-acute care team in the
St. Louis area. This full-time role (5 days a week) involves
rounding in skilled nursing facilities, offering excellent support
and training, autonomy, and significant earning potential. The
anticipated salary ranges from $301,000 to $358,000 annually, with
no limits on productivity-related income. In this position, you
will deliver high-quality care to a diverse patient demographic,
work alongside an experienced multidisciplinary team, and foster
meaningful connections with patients and their families. You will
have the chance to provide exceptional care during critical moments
in their lives. Our comprehensive clinical and operational
leadership team will assist you in transitioning into our medical
practice through a thorough 3-month onboarding program, paired with
a dedicated clinical field mentor and support from leading experts
in post-acute medicine. There are also opportunities for growth and
leadership within this market. Our innovative population health
data reports will help you provide optimal and timely care for our
vulnerable patient population. This medical practice is part of an
Accountable Care Organization (ACO) that participates in the
Medicare Shared Savings Program, which may allow you to receive a
share of the savings if specific criteria are met. Our Medical
Director Leadership Academy (MDLA) will equip you for success in
quality improvement initiatives and collaboration with facility
leadership as a valued team partner. Opportunities for medical

Responsibilities Conduct thorough patient assessments, including
medical histories and physical examinations. Create, implement, and
modify individualized care plans tailored to patient needs. Track
patient progress and coordinate care to achieve optimal outcomes.
Work closely with physicians, nursing staff, and other healthcare
professionals in a collaborative manner. Educate patients and their
families on health management and preventive care strategies.
Maintain accurate and timely medical records in accordance with
regulations. Qualifications Valid physician license in the State of
Missouri and DEA registration. Experience in post-acute, acute,
emergency, or clinic settings is preferred but not mandatory; new
graduates are encouraged to apply. Strong clinical foundation and
understanding of healthcare regulatory standards. Excellent
