Job Description:
SSM Health, a prominent leader in Catholic, not-for-profit
healthcare, seeks a dedicated RN Home Health Full Time to extend
compassionate, mission-driven care within patients' homes across
the Midwest. This role is ideally suited for someone passionate
about providing direct patient care in a dynamic in-home setting,
enriching lives and fostering independent living while aligning
with our integrated health systems's deep-rooted values and focus
on comprehensive health services. Assess patients' conditions
during every visit and chart your observations. Administer
medication as prescribed by the patient's physician. Provide wound
care, IV care, and other direct nursing care as per patient needs.
Educate patients and their families on proper home health care
strategies and procedures. Coordinate with doctors and other
healthcare professionals in regard to patient treatment plans.
Document and report any changes in patient's conditions. Assist in
pain management or other symptom management as necessary. Strong
ability to provide clinical nursing care. Excellent interpersonal
and communication skills. Critical thinking and problem-solving
skills. Proficient with medical software and documentation skills.

401k Current RN license in the state of practice. Valid driver’s
license and reliable transportation. CPR certification
required.
