Job Description:

As a PRN Home Health RN, you will be instrumental in providing
top-tier medical care in the comfort of the patient's home. You
will uphold the values of our Catholic, not-for-profit health
system by delivering personalized health services, and integrating
our fully evolved healthcare delivery system into every patient
interaction. This is a position for individuals seeking an
adaptable schedule with variable hours in various home settings
within the regional areas we serve. Provide comprehensive and
personalized home health care to patients. Collaborate with
physicians and healthcare teams to deliver effective care plans.
Administer medications, manage IVs, and monitor patient recovery
and progress. Educate patients and their families on treatment
plans and home health management. Maintain accurate patient records
and documentation for treatment procedures. Adapt care techniques
to home settings and provide guidance on non-medical management.
Uphold the standards and values of SSM Health in every aspect of
patient care. Strong clinical skills with a thorough understanding
of home health care. Excellent interpersonal and communication
skills. Proficiency in using healthcare databases and electronic
health records (EHR). Ability to work independently and manage time
effectively. Medical Insurance Dental Insurance Vision Insurance
Life Insurance 401k Valid Registered Nurse (RN) license in the
state of practice. CPR certification required. Driver’s license and
reliable transportation as this position requires travel within the
region.
