Job Description Salary: $15 DOE Position Title: Caregiver/Home
Health Aide Reports To: Case Manager FLSA Status: Non-exempt
Department: Care Team Position Summary: Under the supervision of
the Case Manager, follows care plan established for each individual
client. Assists clients in dressing or undressing, bathing, or
eating. Enables clients to maintain good personal hygiene, carry
out daily activities, and maintain good overall health. Documents
client care and services rendered. Essential Duties include the
following: The following duties are normal for this position. This
list is not to be construed as exclusive or all-inclusive. Other
duties may be required and assigned. Assists clients in their
personal care which includes direction and assistance in grooming,
hygiene, dressing, nutrition, etc. Bathes, dresses, and undresses
clients. Encourages and assists clients to participate in
activities. Listens to and understands the problems of clients and
reports observations immediately to Case Manager, if appropriate.
Transports residents or assists clients to walk. Turns and
repositions clients, alone or with assistance, to prevent bedsores.
Changes bed linens, runs errands, directs visitors, and answers
telephone. Takes and records temperature, blood pressure, pulse and
respiration rates, and food and fluid intake and output, as
directed. Assists with laundry and housekeeping duties. Dusts and
cleans. Maintains compliance to all personnel policies, established
policies and procedures, and Federal and State regulations and
standards. Dependable and able to meet schedule requirements
(attendance and punctuality) with no restrictions. Other duties as
assigned by Case Manager. Supervisory Responsibilities : None
Qualifications, Education and/or Experience: To perform this job
successfully, an individual must be able to perform each essential
duty satisfactorily. The requirements listed below are
representative of the knowledge, skills, and abilities required.

aide experience preferred. High School diploma or equivalent
preferred. Must possess a general knowledge of laws and
restrictions regarding senior care. Must exhibit and promote a high
level of customer service, hospitality, curiosity and friendliness
towards all clients, families, visitors, and other employees.
Computer Skills: Technologically savvy. Basic ability of using
Google Chrome, GSuite, and other softwares, a plus. Benefits: PTO

Weekly Pay Holiday Pay Physical Demands: The physical demands
described below are representative of those that must be met by an
individual to successfully perform the essential functions of this

performing the duties of this job, the caregiver is regularly
required to stand and walk. The caregiver is frequently required to
use hands to finger, handle, or feel; reach with hands and arms and
talk or hear. The caregiver is occasionally required to stoop,
kneel, crouch, or crawl. The associate must regularly lift and/or
move up to 50 pounds independently and up to 200 pounds with
assistance of a second team member and/or mechanical devices. Must
be able to push up to 350 Ib. (wheelchair). The caregiver may
occasionally be required to lift greater than 200 pounds with the
assistance of additional staff and/or a mechanical lift. Specific
vision abilities required by this job include close vision and
distance vision.
